Boarding at Jog Lantana Animal Clinic is available exclusively to established JLAC clients and patients with a current veterinarian-client-patient relationship.

Submitting this form does not request, reserve, or confirm boarding. All boarding reservations are subject to availability and confirmation by our team.

CLIENT, PETS, AND STAY INFORMATION

Check in is after 3pm. Pets are welcome to arrive prior to 3pm and a ½ day fee will be applied.
Check out is before 1pm. Pets are welcome to stay past 1pm and a ½ day fee will be applied (pets receiving a bath will not be assessed this fee and are encouraged to go home after 3pm).
Pick-up after 1:00 p.m. may incur additional fees. Pick-up is not available on Sundays or holidays.
Please provide any special contact instructions for your pet’s stay, including the name and phone number of another person we may contact if needed.

FEEDING AND COMFORT INSTRUCTIONS

Please include the food name, amount per meal, feeding times, and any special dietary instructions. If more than one pet is boarding, clearly identify the instructions for each pet.

Clinic food: If food is not provided from home, your pet will receive Royal Canin Gastrointestinal Low Fat. An additional fee will apply when clinic food is provided.

If My Pet Is Not Eating, May We Use Clinic-Approved Flavor Enhancers?

BOARDING SPA BATH PACKAGE

Required for dogs boarding two nights or more: Dogs staying for two nights or longer will receive a Boarding Spa Bath Package before discharge so they can go home clean and comfortable.

The package is based on the pet’s weight and includes shampoo, conditioner, blow-dry, nail trim, ear cleaning, and gentle cleaning around the eyes. When appropriate, excess hair around the eyes may be carefully trimmed to improve cleanliness and comfort.

This service provides routine external cleaning only. It does not include diagnosis or treatment of eye, ear, skin, or other medical conditions. If a medical concern is identified, our veterinary team will contact you before providing additional treatment.

Exceptions may be considered on a case-by-case basis for cats and pets with special needs.
